For Board Determination:

 

I-PSY-2009-9        Documentation submitted by R in response to an audit request shows that R obtained 4.5 hours of approved CE during the audit period and 18 hours of approved CE  outside the audit period. R subsequently provided documentation verifying completion of adequate carry-over hours to satisfy CE requirement in audit period.  IBOL recommends the Board authorize closure.

 

I-PSY-2009-14     C alleged R misdiagnosed C and worked with Rís psychiatrist to put C on medication that made C's problems worse.  A review of the case revealed R provided services to C over 12 years ago, and R's license has been expired for over 3 years.  The Cognizant Memberís review found that it appears C is attempting to hold R responsible for the psychopharmacological assistance provided by a consulting psychiatrist, which seems to be more of an issue for the medical board to evaluate the appropriateness of the psychiatrist who may have prescribed medication without interviewing C.  The Cognizant Member noted that it may be impossible to obtain records from R or the agency since the records only needed to be retained for 5 years.  IBOL recommends the Board authorize closure.

 

I-PSY-2010-4        C alleged C has made three requests for a receipt for payments for therapy but R will not provide a receipt.  The Cognizant Memberís review determined that it may be prudent for the IBOL attorney to write a letter to R indicating that a complaint has been filed and requesting a receipt be sent to C with acknowledgement for the reason of the delay and an apology.  If R is unwilling to comply with IBOLís request, the Cognizant Member recommended that the case be investigated to determine if R is in violation of the APA Code of Ethics and R would be responsible for the cost of the investigation.  IBOL requests direction from the Board regarding the Cognizant Memberís recommendation.
