COMPLAINT INVESTIGATION REPORT

FY2007: To date, one (1) complaint has been received and is under investigation.

FY2006: Three (3) complaints were received.  Two (2) have been closed; and one (1) remains under investigation.

FY-2005: Eleven (11) complaints were received. Seven (7) have been closed; one (1) remains under investigation; two (2) have been referred for legal review; and one (1) awaits Board determination.

FY-2004: Nineteen (19) complaints were received. Nine (9) were closed; eight (8) were referred for legal review; and two (2) remain under investigation.

For Board determination:

ARC-B6A-0-2005-2                R self disclosed discipline on 2004 renewal form. INV revealed that R was disciplined in another state and a Findings of Fact, Conclusions of Law, and Order were obtained.  The Final Order notes that R R prepared drawings for a commercial business and submitted plans to building official; that R falsified the state's application for license; and that R was not licensed in the other state, nor did R's firm have a Certificate of Authorization, prior to preparing and submitting the drawings. R was ordered to pay $15,000 fine and obtain the required licenses. INV confirmed that all conditions of the order have been met and R is currently licensed in the other state. IBOL recommends the Board authorize closure with advisory, which CM confirms.
