Licensure Application Process
Idaho law requires the licensure of all persons who practice or offer to practice physical therapy in the State of Idaho. Licensure as a physical therapist or physical therapist assistant is available through either endorsement or examination.
Applicants for licensure as either a physical therapist or physical therapist assistant must file application with the board. All applicants must provide documentation of their education and proof of graduation from a nationally accredited school of physical therapy approved by the board and a detailed summary of any other qualifications relevant to licensure. Applicants are also required to disclose any criminal convictions other than minor traffic infractions, and any disciplinary actions or licensure denials from any professional regulatory agency, in Idaho or any other state. The application form must be complete and include all requested information and the required fees before it will be considered by the Board.

In situations where the processing of an application requires extraordinary
expenses, the Board may charge the applicant reasonable fees to cover all or
part of the extraordinary expenses.
