License Renewal
Application to renew your
license must be made before your birth date each year in order for your license
to remain valid. You bear the total responsibility of insuring that a timely
and complete renewal application is submitted. Failure to
submit a completed
application to renew your license before the expiration date
will result in the cancellation of your license.
The most efficient way to
submit your renewal application and payment of fees is through the
ONLINE
renewal system. There are no additional fees for this service.
License renewal
applications are mailed approximately six weeks before the license expiration
date. This renewal notice is sent to your mailing address of record and
MUST BE COMPLETED AND RETURNED WITH PAYMENT. If you do not receive a
renewal notice, you may contact the Bureau
to confirm your mailing address and have a duplicate notice sent to you. Failure
to receive a renewal form does not exempt you from the renewal requirement.
Do not wait until the last minute to request a duplicate.
When submitting your
renewal application to the Bureau office, you must include responses to all
questions, your social security number, your signature and the appropriate fee(s)
before the expiration of your license. Do not send cash through the mail.
Returned checks will be subject to a $20.00 collection fee.
The notice for a FUNERAL ESTABLISHMENT (FE) license must be signed by the resident mortician and returned with the appropriate fee to the Bureau office to renew the license.
Please note that CREMATORY (CRE) licenses will not be renewed until the appropriate funeral establishment license has been renewed.
CERTIFICATES OF AUTHORITY (MCA) will not be renewed until the appropriate mortician license has been renewed. MCA renewal requests must include the annual Certified Audit Report as required by § 54-1134.C., Idaho Code.
NOTE: If you fail to renew your license before the expiration date, your license WILL be canceled. Renewals postmarked on or after the expiration date must include a $250.00 reinstatement fee in addition to the regular renewal fee. The reinstatement fee is required by §67-2614, Idaho Code and cannot be waived. All questions regarding the reinstatement fee must be submitted by letter or e-mail. Phone calls regarding this matter will NOT be returned.
Continuing Education
Continuing education is not required to maintain a license to practice as a mortician in Idaho.
