License Renewal
Application to renew your
license must be made before your birth date each year in order for your license
to remain valid. You bear the total responsibility of insuring that a timely
and complete renewal application is submitted. Failure to
submit a completed
application to renew your license before the expiration date
will result in the cancellation of your license.
The most efficient way to
submit your renewal application and payment of fees is through the
ONLINE
renewal system. There are no additional fees for this service.
License renewal
applications are mailed approximately six weeks before the license expiration
date. This renewal notice is sent to your mailing address of record and
MUST BE COMPLETED AND RETURNED WITH PAYMENT. If you do not receive a
renewal notice, you may contact the Bureau
to confirm your mailing address and have a duplicate notice sent to you. Failure
to receive a renewal form does not exempt you from the renewal requirement.
Do not wait until the last minute to request a duplicate.
When submitting your
renewal application to the Bureau office, you must include responses to all
questions, your social security number, your signature and the appropriate fee(s)
before the expiration of your license. Do not send cash through the mail.
Returned checks will be subject to a $20.00 collection fee.
On July 1, 1999, the Board adopted an emergency rule which instituted an inactive license category. Those licensees with a current license may elect to have their license converted to Inactive status by notifying the Board in writing and submitting a fee of $50.00. An Inactive license may be renewed annually for up to five years by submitting the $50.00 renewal fee and proof of meeting the continuing education requirements. An Inactive license may be re-activated at any time by paying the required renewal fee for an Active license and submitting proof of having met the continuing education requirements for the current year.
FEES
Annual renewal fee $300.00
Inactive License fee $50.00
Application/Examination fee $300.00
Continuing Education
The annual completion of 12 hours of Board approved continuing education is required to maintain a license to practice denturitry in Idaho. Official documentation of attendance must be submitted with the renewal form. The Board has determined that correspondence courses and home study will not meet the continuing education requirement. If you have questions about the CE requirements, you may e-mail the Bureau office.
CE providers may obtain Board approval by submitting a completed Continued Education Approval Form.
The Board suggests that all applications for approval or pre-approval of course work be submitted prior to the April board meeting. Timely submissions will allow adequate time for the Board to review the materials and provide notification prior to renewal time.
