Licensure Application Process

Idaho law requires the licensure of all persons who practice or offer to practice denturitry in the State of Idaho. Licensure is available through both endorsement and examination.

Any licensed denturist who operates a denturist lab or clinic, or who conducts business as a denturist, must register their business location with the board annually (See Rule 475).  The DENTURIST ANNUAL BUSINESS REGISTRATION STATEMENT form for Denture Clinics must be completed and submitted to the Board within ten (10) days of opening for business. The form must also be submitted for any change in either location, identity of the principal denturist, or of any denturist employees. If you operate or conduct business at more than one business location, please copy and complete this form for each business location.
