                  SUBJECT:        Complaint Investigation Report   

Fiscal Year 2005: No complaints have been received to date.

Fiscal Year 2004: Eight (8) complaints were received. Of those, two (2) were closed without action; two (2) were referred for disciplinary action; three (3) remain under investigation; and one (1) awaits Board determination.   

Fiscal Year 2003: Four (4) complaints were received. Of those, three (3) were closed without action, and one (1) remains open and under investigation.   

DEN-S5B-2-2004-9            C alleges that R billed Medicaid for denture services and merchandise that were never delivered. INV revealed that, though dentures were made and Medicare billed, the dentures were never picked up by the client. Records confirmed that client had been called repeatedly, and that Medicare had been refunded all payments. INV did not reveal evidence of a violation.
The Bureau recommends the Board authorize closure.
