Idaho State Cosmetology Board meeting was called to order at 1:35 p.m., Monday,
June 28, 2004 at the Bureau of Occupational Licenses, Owyhee Plaza, 1109 Main
Street, Suite 220, Boise, Idaho.

DISCIPLINARY COMPLAINTS
COS-L6A-02A-04-39:
Moved by Joan Huff, seconded by Linda Swope and carried to accept the
recommendations of the Deputy Attorney General.
COS-B1B-02-04-56:
Moved by Joan Huff, seconded by Linda Swope and carried to accept the
recommendation of the Deputy Attorney General.
COS-B4-2-04-54
and COS-B4-2-04-55: Moved by Joan
Huff, seconded by Linda Swope and carried to accept the recommendation of the
Deputy Attorney General.
FORMAL DICSIPLINARY ACTION
COS-2004-13:
Moved by Joan Huff, seconded by Linda Swope and carried that the
cosmetology license of respondent be suspended for a period of five (5) years
and shall be prohibited from operating a cosmetology establishment for a
five-year (5) period. Respondent
shall pay a One thousand dollars (1,000) fine and pay all cost and attorney fees
incurred in the investigation and prosecution.
COS-2004-14, COS-2004-16, and COS-2004-17:
Moved by Joan Huff, seconded by Linda Swope and carried to deny the
request for Respondents Joint Petition For Reconsideration of the Final Orders.
Motion made by Ms. Huff to raise the school
Bond to $25,000, seconded by Ms. Swope, motion carried.
Motion made by Ms. Huff to discontinue the
apprenticeship program, seconded by Ms. Swope; motion carried.
Motion made by Ms. Huff to change the
student instructor ratio from 1 to 15 to 1 to 20 with the caveat that the
student instructor does not count as a student in that ratio, seconded by Ms.
Swope; motion carried.
Motion made by Ms. Huff to amend the
student instructor ratio to include that the instructor count as an instructor
and not as a student, seconded by Ms. Swope; motion carried.
Motion made by Ms. Huff to table the
discussion on the requirements of the demonstration permit until the October 4,
2004 board meeting, seconded by Ms. Swope; motion carried.
Motion made by Ms. Huff to table the
discussion on the requirements for the appointed examiners until the October 4,
2004 board meeting, seconded by Ms.
Swope; motion carried.
