The Idaho State Cosmetology Board conference call was called to order at 8:09 a.m., Wednesday, May 26, 2004 at the Bureau of Occupational Licenses, Owyhee Plaza, 1109 Main Street, Suite 220, Boise, Idaho.

Moved
by Joni Huff, seconded by Linda Swope, and carried that the Board go into
executive session to discuss disciplinary matters.
Moved
by Geneal Thompson, seconded by Joni Huff, and carried the Board come out of
executive session.
Formal Disciplinary Action
The Board reviewed the Findings of Fact, Conclusions of Law, and Recommended Order for COS-2004-14, COS-2004-16, and COS-2004-17.
COS-2004-14:
Moved by Geneal Thompson, seconded by Linda Swope and carried that the
respondents request to vacate the default order and remand the matter back to
the hearing officer has been denied, that respondent’s license be suspended
for a period of one hundred twenty (120) days with ninety (90) days stayed,
license shall be placed on probation for a period of three (3) years, respondent
shall pay a one thousand dollar (1,000) fine and pay all investigative and legal
costs incurred in the investigation and prosecution in this matter.
COS-2004-16:
Moved by Joan Huff, seconded by Geneal Thompson, and carried that the
respondents request to vacate the default order and remand the matter back to
the hearing officer has been denied, and that respondent’s establishment
license be suspended for a period of one hundred twenty (120) days with ninety
(90) days stayed, license shall be placed on probation for a period of three (3)
years, respondent shall pay a one thousand dollar (1,000) fine, and pay all
investigative and legal costs incurred in the investigation and prosecution in
this matter.
COS-2004-17:
Moved by Joan Huff, seconded by Linda Swope and carried that the
respondents request to vacate the default order and remand the matter back to
the hearing officer has been denied, and that respondent’s license be
suspended for a period of one hundred twenty (120) days with ninety (90) days
