APPLICATION FOR COSMETOLOGY ESTABLISHMENT LICENSE

INSTRUCTIONS

Every establishment wherein cosmetology, nail technology, esthetics, or electrology is practiced must be licensed.  An establishment may be licensed as a Primary establishment or a Contiguous establishment (see Rule 300).  

Contiguous establishment licenses are issued for salons that are located within existing Primary establishments.  The Contiguous establishment license is issued to the owner of the Contiguous establishment, and identifies said owner as the party responsible for complying with the sanitary and licensure requirements within the Contiguous establishment.  (DISCLAIMER: A Contiguous establishment license does not, in and of itself, constitute the independence of a Primary or Contiguous establishment's business or employment practices.  There are specific state and federal tax and employment obligations and requirements concerning the operation of a business.  For more information contact: the Idaho Department of Labor; the Idaho Tax Commission; the Idaho Industrial Commission; the Internal Revenue Service.

All establishment applications must include an accurate and detailed floor plan of the entire establishment area, drawn on a separate sheet of letter size 8 and 1/2 inch by 11 inch white paper. The floor plan must include: all inside dimensions, total square footage, location of all stations, water sources, restrooms, access areas, and entrances. If the establishment is located within a multi-tenet building or a private residence, please include a drawing of the complete building or residence showing all surrounding or adjacent rooms and the exact location of the shop area within the building or residence. The floor plan must include the exact measurements of the entire area to be licensed.

CAUTION: Establishments located within a residence must have a separate outside entrance leading directly into the licensed area.

THE APPLICATION WILL NOT BE PROCESSED IF IT IS NOT COMPLETE.  THE COMPLETED APPLICATION MUST BE SUBMITTED WITH THE REQUIRED FLOOR PLAN & FEE. 

NOTE: The Board has waived the application fee for those Contiguous establishments that change location (station) WITHIN THE SAME PRIMARY ESTABLISHMENT.  Contiguous owners must continue to file the Contiguous Establishment License Application for such changes prior to such a move.   Contiguous establishments that move from one Primary establishment to another Primary establishment are required to submit both the application and the required fee.

You may download either an Original Establishment License application or a Contiguous Establishment License application, or obtain one from the Bureau office.
